This is the date you are submitting the request, not the date you are requesting. The form should be submitted for approval at least 30 days prior to the event, if possible. COM meets on the 4th Sunday of even months. The event approval form should be submitted and approved before dates are finalized and any advertising takes place.
This is time that the event will actually start. Do not include set up time. You can include this when you make the facility reservation.
Do not include time needed for tear down. This is the time that the event will end for participants.
If the event requires fundraising, a separate fundraising approval form should be completed and submitted for approval by COM
Once the event is approved by COM, the person responsible for the event should complete the facility reservation form, which should include set up and tear down time. If you need AV support or childcare, please complete the corresponding forms.
